Community Health Worker –Strengthening Senior Support Networks

.5 FTE– One year contract (with possibility of extension) 

South-East Ottawa Community Health Centre is seeking applications for a Community Health Worker to work with the new three year Strengthening Senior Support Networks project.

The incumbent will use a community development approach to assist seniors living in low income neighbourhoods to strengthen their social networks and improve their access to services.    They will work collaboratively with multiple partners in program planning, assist in evaluation, act as a resource to communities and partner organizations, and provide linkages and resources that support seniors.  The successful candidate will have proven facilitation skills, knowledge of community and health services available to seniors, and have experience working with partners.

Requirements

  • Bachelor’s degree in a relevant discipline or equivalent combination of training and experience.
  • Minimum two years experience in a community-based setting.
  • Minimum two years experience working with seniors.
  • Solid knowledge of community and health resources.
  • Ability to assess situations and to respond appropriately.
  • Ability to maintain records and data requirements using data bases
  • Ability to work collaboratively in an inter-professional setting, with clients and other agencies.
  • Ability to work flexible schedule including some evenings and weekends.
  • Valid driver’s license, insurance and use of a personally owned vehicle.

REGISTERED NURSE – Primary Care Outreach (Seniors)

South-East Ottawa Community Health Centre is seeking applications for a Registered Nurse for outreach nursing activities with seniors in their homes, primarily out of our Western Ottawa Community Resource Centre Site. The successful candidate will work as a member of an inter-professional team to deliver high quality Primary Health Care Outreach services to seniors.

Requirements

  • Bachelor level education in Nursing, registered with the College of Nurses of Ontario
  • Nursing experience in a community-based setting
  • Nursing experience working with seniors
  • Aptitude and interest in client education and health promotion with seniors
  • Knowledge of community resources for seniors.
  • Ability to work independently in the community
  • Strong clinical assessment skills.
  • Valid driver’s license, insurance and use of a personally owned vehicle.
  • Fluency in English required, French and other languages an asset
  • Proficiency in the use of computers and software applications

Salary range: $31.63 to $37.21/hour, Depending on Experience.  (HOOPP Eligible)

FINANCE OFFICER

Full Time Indeterminate (35 hours/week)

Position Description:

South-East Ottawa Community Health Centre is a non-profit health, community and social services agency. The Centre requires a Finance Officer to provide professional accounting duties related to payroll, purchasing and accounts payable services, and to assist the Director of Finance and Corporate Services in establishing and maintaining an organized set of records and statements. 

Qualifications: 

  • Diploma or certificate in related field, preferably Commerce, Business Administration, Human Resources or Accounting.
  • Minimum 5 years’ relevant experience, preferably in a not-for-profit organization. Experience with computerized financial information systems, including a sound knowledge of automated bookkeeping, payroll and accounting systems
  • Proficiency with computerized accounting systems and spreadsheets, specifically, Clarity, MS Dynamics: GP and HOOPP
  • Familiarity with word processing and communication software in a WAN environment.
  • Demonstrated ability to prioritize, to be flexible and to meet deadlines.
  • A good grasp of accrual based accounting.
  • Good communication and interpersonal skills.
  • Excellent English skills, spoken and written; French and other languages an asset

Salary Range: $46,436 to $54,630/annum DOE, plus benefits and Health Care of Ontario Pension Plan
