 South-East Ottawa Community Health Centre is an inter-professional health, social services and community services agency. We are seeking a bilingual Service Coordinator in Community Support Services(CSS). Salary Range: $44,371 to $52,201/year

 JOB SUMMARY

Reporting to the Seniors Team (ST) Coordinator, the Service Coordinator coordinates community support services for seniors, adults with physical disabilities and caregivers.

 RESPONSIBILITIES

  • Assess clients to determine their needs and preferences for appropriate services, informs them of available internal and external programs and services, and makes appropriate referrals. Internal CSS include: transportation to medical appointments, grocery bus, friendly visiting, handy helper program, caregiver support, LunchAbility, crisis intervention & assistance, foot care, and a directory of workers for homemaking, home maintenance and respite care,.
  • Conduct home visits for clients as needed and keep up-to-date client records.
  • Liaise with community partners to ensure effective provision of service delivery
  • Coordinate and monitor worker/volunteer matches with clients and do follow-ups
  • Assist with advocacy, recruiting and interviewing of volunteers and workers
  • Support program development, promotion & evaluation.

 QUALIFICATIONS:

  • Post-secondary education in Social Sciences;
  • Previous related experience in home support services;
  • Experience working with seniors and with adults with physical disabilities.
  • Fluency in English and in French
  • Access to a car to travel to clients’ homes in south-east Ottawa.
  • Strong computer skills, experience with client database.
  • Excellent attention to detail
  • Excellent telephone manner/skills

Family Physician

SEOCHC is seeking a part-time physician to join our interdisciplinary primary care team of family physicians, nurse practitioners, nurses and other professionals.

Requirements:

  • Family Physician licensed by the College of Physicians and Surgeons of Ontario and certified by the College of Family Practice of Canada.
  • Experience in primary care, including community health, hospital and/or family practice desirable.
  • Sensitivity to the needs of seniors and lower income multicultural groups.
  • Excellent communication skills in English (written and verbal)
  • Ability to provide service in French and/or other languages an asset
  • Familiarity with PS Suites strongly preferred

Salary range:  $183,445 to $212,438 pro-rated to FTE (generous benefits and HOOPP eligible)

MEDICAL SECRETARY 

Relief, Salary Range $20.48 to $24.38/hour

South-East Ottawa Community Health Centre is a non-profit health, community and social services agency. The Centre requires a Medical Secretary to work with our busy Health and Social Services Administrative Team which provides support to multiple health care providers.

The Medical Secretary’s primary role is to make clients of the Centre welcome and to facilitate smooth access to programs and services by providing medical administrative support.  Duties and responsibilities include answering phone lines, greeting, registering and scheduling clients, arranging referrals as required and providing administrative support as needed such as photocopying, scanning, faxing, filing, etc. The Medical Secretary is also responsible for implementing the Centre’s policies, procedures, client reception and administrative protocols.  Extended clinic hours require flexible work schedule.

QUALIFICATIONS

Post-secondary training in office management, medical secretarial or a related program.

  • Two years’ experience in a Health/Social Service Setting, office administration, preferably in a multi-disciplinary setting and a not-for-profit organization.
  • Fluent oral and written skills in English and French preferred; other languages an asset.
  • General knowledge of medical terminology
  • Proficiency in word processing (min. 50 wpm), data entry, electronic scheduling, records management, PS Suites electronic medical records.
  • Experience working with marginalized communities
  • Excellent communication and interpersonal skills, including conflict resolution, problem solving.
  • Proven team-work skills and ability to multitask.
