Become a Patient

SEOCHC’s Primary Care Clinic is accepting new patients. A family doctor or nurse practitioner at SEOCHC can look after your health, not just for one visit, but over time.

There are two ways to ask to become a patient. Read both steps below to help you choose the right one for you.

Step 1: Register with Health Care Connect

For most people, this is the first step. Health Care Connect is a free program from the Government of Ontario. It helps people find a family doctor or nurse practitioner. It may match you with one at SEOCHC or at another clinic close to your home.

You can register in two ways:


Step 2: Register directly with SEOCHC (only in some cases)

Some people should register with SEOCHC directly. Use SEOCHC’s form if one or more of these is true for you:

  • You cannot use Health Care Connect.
  • You tried Health Care Connect, but it is not working for you.
  • You want to become a patient at SEOCHC, and you are eligible to become a SEOCHC patient.

What happens next?
A member of our Primary Care Intake Team will review your information. They will contact you within 2 to 3 weeks to talk about the next step.

Please note
Sending the form does not automatically mean you will become a patient. We review every request and look at three things: who is eligible, who needs care the most, and how many new patients we can accept.
