SEOCHC is excited to introduce our new Nurse Practitioner-led Walk-In Clinic, a new initiative dedicated to providing accessible, culturally responsive and compassionate care for priority populations. Whether you’re seeking care for acute symptoms,  or dealing with a minor illness or injury, or need support with your overall health, our staff are here to provide expert care when you need it most! No appointments needed!

Services Offered

Diagnosis & treatment of minor illnesses/injuries

• • Forms, Notes & Prescription Renewals

• • Sexual Health Services

• • Vaccinations & Immunizations

• • Preventative Care & Screening

• • Mental Health Support – Referral to Specialists

Eligibility Criteria:

• • Live within SEOCHC catchment

• • Need occasional care

• • Do not have a primary care provider (Nurse Practitioner or Doctor)

• • Member of a priority populations (African, Caribbean, Black, low-income, LGBTQ+, Newcomers, Refugees, etc.)

• • Facing barriers to accessing care

Hours of Operation: Tuesdays 12PM – 7 PM & Thursdays 9AM – 4PM
