My name is Chantal Hoffman, and I am a Registered Midwife. This clinic provides short-term prenatal care while we help connect you with a midwifery practice group or obstetrician who will be your main care provider during pregnancy and birth. While you are waiting for that referral to be accepted, you are welcome to continue receiving care here.
Your privacy is very important to us. Everything we discuss will remain confidential, except in cases where there is a risk of serious harm to yourself or others—in such cases, I am legally required to report.
I will only share your contact and health information for the purpose of helping you find a long-term care provider, and only with your permission.
Your personal information will never be shared with immigration authorities or government agencies.
Midwives in Ontario are funded to care for people without OHIP or IFH. This means that your visit with me is free.

I may ask you questions about your residency status and income. These questions are optional, but your answers help us connect you with the most appropriate resources—such as social assistance programs that can cover prescription costs.
